Our work follows a five-stage chain. Each stage builds the record the next depends on. The sequence is the point. Skip a stage and the finding weakens. Rush the documentation and the recovery fails on appeal. We treat the chain as a discipline, not a diagram.

Identification
Purpose-built detection technology analyzes claims data at scale. It surfaces anomalies, patterns, and outliers consistent with fraud, waste, abuse, and process failure, and it separates true signal from statistical noise before any allegation is formed. We do not start with accusations. We start with evidence of where to look.

Analysis
Each flagged pattern is examined in context. Provider behavior, billing history, policy, and program rules all inform the question that matters: what is this anomaly, and does it rise to a finding? Patterns that do not survive analysis are set aside. Patterns that do move forward with their context attached.

Documentation
Findings are recorded to an evidentiary standard: sourced, reproducible, and structured to hold under appeal, administrative challenge, and legal scrutiny. Documentation is not a report written at the end. It is built as the work happens, so the record and the finding are the same thing.

Recovery Support
We support recovery of improper payments through the channels appropriate to your program. Recoveries get appealed and challenged. Ours are documented from the first finding to hold when they are.

Deterrence and Monitoring
Findings feed forward into our own monitoring and detection technology, so the same leak does not reopen. Your posture shifts from reacting to loss toward preventing it. The chain does not end with a recovery. It ends with the reason you will not need the same recovery twice.

The Standard
Findings you cannot defend are liabilities wearing the costume of results. We document to the standard of the hardest room our work could end up in, because for the entities we serve, it often ends up there. If a finding cannot be defended, it is not reported.
